Before we begin the intensive, there are a few important items to complete.


These steps help me prepare for our time together and help you arrive focused, organized, and ready to begin.


The first three items on this page should be completed at least 7 days before Day 1 whenever possible.

The intake form provides important background information about you, your relationship, your family, and the challenges that have brought you here.


This information helps me prepare before we meet so we can make the best use of our time together.

Please complete and submit the intake form at least 7 days before Day 1.

Tell Me Your Story


Each partner is invited to send me an email sharing their perspective on the relationship.


Write whatever feels important for me to understand before we begin.


If you're not sure where to start, consider sharing:

  • What brought you to Relationship 911?
  • What concerns you most right now?
  • What do you feel has happened in the relationship?
  • What would you most like to see change?
  • What would a successful intensive look like for you?
  • What are you hoping to take away from our time together?


There are no right or wrong answers.


This is simply an opportunity for me to better understand your experience before Day 1.

Complete Your Payment


If you have not already paid in full, the remaining balance is due at least 7 days before Day 1.


This allows us to finalize preparation and focus our attention on the work ahead.


If you need to discuss arrangements or have questions about your balance, please contact me as soon as possible.

You may find it helpful to decide who will take responsibility for each item and when it will be completed.
